245D Licensed Services

Minnesota's 245D licensing ensures that providers of home and community-based services meet rigorous standards for quality, safety, and person-centered care. Clemens Health holds a 245D license, which means we have been evaluated and approved by the Minnesota Department of Human Services to provide disability services. This license covers our community living support, individualized home supports, and respite care services. For families, a 245D license is your assurance that the provider has trained staff, proper supervision, individual service plans, and accountability measures in place.

Eligibility

  • Individuals receiving services through a Medical Assistance waiver program
  • People with developmental disabilities, brain injuries, or mental health conditions
  • Individuals whose care plan requires services from a licensed 245D provider
  • People referred by their county case manager or care coordinator

Covered services

  • Respite care (in-home and out-of-home)

How to apply

  1. Talk to your case manager or care coordinator about which 245D services are right for you.

  2. Your case manager will help identify the services in your person-centered plan.

  3. Request Clemens Health as your 245D provider. Your case manager can make the referral.

  4. We complete an intake process that includes reviewing your service plan and meeting your family.

  5. Services begin according to your authorized plan, with a support team assigned to your case.
